REGISTER FOR ORIENTATION: If you haven’t already done so, make sure you and your parents register for Orientation.
Immunization Forms: These are REQUIRED in order to complete registration for classes. Students should submit their immunization forms ASAP, not waiting until they arrive on campus. Please fax Immunization Forms directly to the Student Health Center (941-487-4256) and put the original copies in the mail to us.
Academic Advising Questionnaire (AAQ): This form helps us determine your faculty advisor, so let us know about your academic interests. This is non-binding – it’s just meant to be helpful to us, and to you.
Course Request System (CRS): Entering your desired courses here shows us what you are interested in. You are not registering for courses here (you do that on campus during Orientation in August). This system helps us determine the number of sections needed for potential “high demand” classes.
Housing Preference Form: Help us, help you! Fill out this form indicating your roommate preference. We can’t guarantee you’ll get exactly what you want, but this will hopefully help you transition to college living with ease.
Roommate Matching Form: This form helps the staff try to assign you a compatible roommate. Be honest and thoughtful with your answers.
FERPA Release Waiver: This waiver gives consent to allow or not allow others to view personal information. Please read this form carefully. **Required by federal law**
Parent Contact Information: The Business Office keeps this on file for billing and contact purposes. **Required to be kept on file**
Title IV Authorization: If you have financial aid, this form must be completed. You must state, via this form, how your financial aid can be used.
Transcripts – Final for high school: For students who only attended high school.
Transcripts – Final for dual enrollment: For students who were officially dual enrolled in high school and college
Transcripts – final for college transfer: For students who are transferring from another college (not high school/dual enrollment).
Don’t forget to make your hotel and/or travel arrangements for you and your family! Visit our accommodations section for information about local hotels.
While you’re at it: plan ahead! Tell your family to schedule a visit during Family Weekend – September 27-29, 2013. Hope to see your family there!
