STUDENT HEALTH SERVICES/IMMUNIZATIONS

Medical Services:
Student Health Services creates a medical file
for each incoming student. It is important that information
regarding immunization and health insurance be accessible to you in
the event of an emergency. Please
provide documentation
of immunizations to the Counseling and Wellness Center at Parkview House
for clearance to register for classes.
Also, measles and MMR (measles, mumps,
rubella) shots
are available during the Fall & Spring terms for a nominal fee.
Mandatory:

Medical History Form with proof of two
measles immunizations.

Health Information Form.
-
Waiver for Treatment of Minors (if
applicable).

Either proof of Hepatitis B immunization series AND Meningitis
immunizations OR signed waiver form.
Recommended:
-
Date of last Tetanus vaccination.

Copy of Health Insurance card.

Medical records pertaining to continued
medical treatment you may need while on campus
Medical services are available with
nurse practitioner Linda Marsh ARNP, Monday through Friday 2:00 -
5:00 pm during the Fall & Spring terms at Parkview House. Medical services are also available off-campus
with a physician under contract with the college. Students
must bring their New College identification card with them to
be seen for treatment off campus.
Flu shots and immunizations
may be available at both locations for a fee. Standard office visits are free and students
receive a discount on lab services and other tests. For
specific cost information or an appointment,
call (941) 487-4254. Medical services are not available
during Summer term or Winter break.
